  • Abstract
    Epstein-Barr virus(EBV)is the pathogen that most commonly triggers infection- associated hemophagocytic lymphohistiocytosis (HLH) and ectopically infects CD8+ T cells in EBV-associated HLH(EBV-HLH). We recently described an EBV-HLH patient who had a clonally expanded population of EBV-infected CD8+ T cells with CD5down-regulation. To determine whether this finding could serve as a useful marker for EBV-HLH, we investigated5additional patients. We found a significant increase in the subpopulation of CD8+ T cells with CD5 down- r e g u l a t i o n a n d b r i g h t h u m a n l e u k o c y t e a n t i g e n ( H L A )- D R e x p r e s s i o n i n a l l patients with EBV-HLH but not in patients with infectious mononucleosis or in control subjects. Such T cells were frequently found to be larger cells that stained positive for a specific T cell receptor VB. We also demonstrated that those cells were the major cellular target of EBV, and their numbers progressively declined in parallel with the serum ferritin levels. All together, our findings reveal the immunophenotypic characteristics of EBV-infected CD8+ T cells and may provide a valuable tool for the diagnosis of EBV-HLH.

  • Paradoxical inhibition of basophil degranulation by excess binding of a cedar pollen allergen, Cryj1.

  • Abstract
    「Abstract」Background : Repeated exposure to allergens leads to tolerance in type I allergy. However, its mechanism is largely unknown at cellular level. Basophil stimulation test serves as a sensitive method to evaluate allergen- specific activation of basophils. It has been reported that basophil stimulation results in typical bell-shaped dose-response characteristics with inhibition of activation at high concentrations. However, there has been no study evaluating the dose-effect of allergen binding directly. Objectives : To disclose relationships between allergen dose and binding, parameters of basophil activation and severity of the clinical symptoms. Methods : Peripheral blood was obtained from volunteers with proved cedar pollen allergy. Basophil stimulation test was performed with different concentrations of biotin-conjugated cedar pollen antigen, Cryj1. Cryj1 binding, expression of surface antigens, and release of mediators were determined simultaneously. Severity of symptom was evaluated by scoring nasal, eye and throat symptoms and QOL of the subjects. Results : In all cases, bell-shaped dose-response was obtained for the expression of basophil granular proteins LAMP1, LAMP2and CD63on cell surface and release of mediators. In contrast, expression of the surface antigen CD203c and Cryj1 binding increased dose dependently without high-dose inhibition. Basophil activation was induced at very minute level of Cryj1 binding and the differences in dose-response profile reflected the level of inhibition at higher Cryj1 concentration. The levels of inhibition inversely correlated with the severity scores of the clinical symptoms. Conclusions : Basophil activation is induced at very low level of allergen binding and inhibited at high allergen level by individually different manner.

  • Abstract Ectopic Epstein-Barr virus(EBV)infection in natural killer(NK)cells and T cells is associated with hemophagocytic lymphohistiocytosis, chronic active EBV infection(CAEBV)with T/NK cell proliferation, and malignant lymphoma. We newly established seven EBV-infected NK cell lines and two T cell receptor (TCR)γδ+ T cell lines from patients with CAEBV and NK cell leukemia. To identify the cytokines and chemokines produced by EBV+ NK cells or T cells,79 soluble factors were screened in the supernatant of theses cell lines by cytokine antibody array. EBV+ NK cells and T cells and shortly cultured EBV+ NK cells produced a significantly greater amount of four type 1 inflammatory cytokines, namely, interferon-γ, interleukin(IL)-6, oncostatin M, and IL-8, two type 2 cytokines, namely, IL-13 and granulocyte-macrophage colony-stimulating factor (GM-CSF), and three chemokines, namely, I-309/CC chemokine ligand(CCL)1, monocyte chemoattractant protein-1(MCP-1)/CCL2, and regulated on activation, normal T cell expressed and secreted(RANTES)/CCL5, compared to normal activated EBV-negative NK cells or T cells and EBV-positive B cell lines. This accelerated production of multiple cytokines and chemokines in EBV-infected NK /T cells may be related to the pathogenesis of EBV-associated disease with proliferation of EBV-infected NK cells or T cells.

  • Analysis of mutations and recombination activity in RAG-deficient patients.

  • Abstract Mutations in the recombination activating genes(RAG1or RAG2)can lead to a variety of immunodeficiencies. Herein, we report5cases of RAG deficiency from 5families : 3of Omenn syndrome,1of severe combined immunodeficiency, and1 of combined immunodeficiency with oligoclonal TCRγδ+ T cells, autoimmunity and cytomegalovirus infection. The genetic defects were heterogeneous and included6novel RAG mutations. All missense mutations except for Met43Ile in RAG2 were located in active core regions of RAG1 or RAG2. V(D)J recombination activity of each mutant was variable, ranging from half of the wild type activity to none, however, a significant decrease in average recombination activity was demonstrated in each patient. The reduced recombination activity of Met43Ile in RAG2may suggest a crucial role of the non-core region of RAG2in V(D)J recombination. These findings suggest that functional evaluation together with molecular analysis contributes to our broader understanding of RAG deficiency.
